Street Maintenance Request Form Thank you for visiting SDOT's Street Maintenance Pot Hole reporting page. Our goal is to repair potholes within 48 hours (two business days) of receiving a report. Currently our success rate is 94%. Use this form to report non-emergency problems such as pot holes and other minor street repairs, sidewalk safety concerns, malfunctioning traffic signals, and damaged street signs. If you have a concern about vegetation overgrowth, please use this link to get to the appropriate complaint form.
