Request Services

Service Referral Form

A staff member will contact you within two business days after you submit your request.

Provider Schedule

Monday:

  • Catholic Community Services (8:30 a.m.-12:00 p.m.): Coordinated Entry for All Assessments, employment navigation, housing diversion, move-in cost assistance, information & referrals
  • King County Public Health (9:00 a.m.-12:00 p.m.): Basic Food, ORCA Lift Card, DSHS application assistance, health coverage
  • MPS Lifeline Smartphone Access (1:00 p.m.-4:00 p.m.): Free phone with unlimited minutes & text, 15 GB data for 12 months for low-income individuals 
  • CRC Services (all day): Bus tickets, emergency food bags, hygiene kits, clothing, service provider referrals

Wednesday:

  • DSHS (Virtual, 8:30 a.m.-12:00 p.m.): Food, Cash & Medical Benefits, Housing Essential Needs (HEN), ID replacement, child support, mental health & addiction services
  • King County Public Health (9:00 a.m.-12:00 p.m.): Basic Food, ORCA Lift Card, DSHS application assistance, health coverage
  • YWCA (9:00 a.m.-4:00 p.m.): Mental health, eye and dental care, substance use disorder inpatient/outpatient referrals, health coverage, RX and medical bills assistance
  • MPS Lifeline Smartphone Access (9:00 a.m.-2:00 p.m.): Free phone with unlimited minutes & text, 15 GB data for 12 months for low-income individuals 
  • Ideal Option (1:00 p.m.-4:00 p.m.): Medication-assisted treatment for individuals with addiction to all substances
  • CRC Services (all day): Bus tickets, emergency food bags, hygiene kits, clothing, service provider referrals

Thursday: 

  • YWCA (Virtual, 9:00 a.m.-4:00 p.m.): Mental health, eye and dental care, substance use disorder inpatient/outpatient referrals, health coverage, RX and medical bills assistance
  • CRC Services (all day): Bus tickets, emergency food bags, hygiene kits, clothing, service provider referrals
