Adoption questionnaires

If you wish to fill out a digital questionnaire using a the fillable PDF form, you will need to download and save the document before filling it out. You may also print the questionnaire and complete by hand; printed copies are available at the Seattle Animal Shelter. If you are unable to fill out a written questionnaire, staff will assist you at the shelter by conducting a verbal interview that mirrors the written questionnaire. Translation services are also available upon request.

*If you reside in Seattle or King County, you must purchase a pet license at the time of adoption.
High-interest animals may have a higher adoption fee. Contact animal care staff for details.

The adoption fee includes:

  • Spay/neuter surgery for dogs, cats and rabbits.
  • Recommended vaccinations.
  • Microchip and registration.
  • Initial worming.
  • FIV/FeLV testing (cats only).

Gold/FLASH card discount

A 50 percent discount on altered license fees is available for seniors (over 60 years) and people with disabilities who have a Gold or FLASH card.
