New Utility Customers

To reduce the risk of identity theft and comply with the Federal Fair and Accurate Credit Transactions Act (FACT Act)

New customers to the Seattle Utility service area that do not have an established account with any of the City’s utilities, are required to present approved, Government issued picture identification to open an account for any utility service. Learn more >

New Utility Customers should:

  • • Complete a Residential Utility Service Application (PDF)
    Note:
    Save the form to your computer by selecting “Save As” under the “File” drop down menu in your browser. This form must be printed and signed, then mailed, faxed or submitted in person.
  • • Pick the Garbage can size and Food and Yard waste container you need.
  • • Submit the completed and signed Residential Utility Service Application in one of the three ways below:
 
    1. Take the form to a Service Center for processing.

    2. Take the form to a Notary to verify your identification and signature and mail or fax it.

    3. Landlords can verify the renters’ photo identification and signature then, complete and sign the Landlord section and submit it for you in person, by mail or fax.
