Need a Tax Form? Each year, we receive a number of tax filings using non-standard forms or photocopies of prior periods. Each tax return is bar coded at the top of the page with the customer number, reporting period, and obligation number (like an invoice number) for the specific tax period imbedded in the coding. Using a tax return from another period and account will result in a misapplication of funds. If you need a blank tax form for a reporting period, please contact us. We will mail or fax a form the same business day the request is received.
