Benefits Information
Medical
The City offers regularly appointed employees and their eligible dependents the choice between four medical plans, two dental plans and a vision plan. Most employees share in the cost of premiums for the medical plans (unless otherwise identified in a union contract). The City pays the premiums for dental and vision coverage.

- Call the above numbers and order by phone.
- FAX a Mail Order Request Form with credit card information to (206) 901-4443
- Register for MyGroupHealth and order prescriptions on line. Register at www.ghc.org.
