Community Living Connections

The City of Seattle is part of the Community Living Connections network, offering an extensive, reliable network for aging and disability services in the King County area. If you are an older  adult needing help with transportation, helping someone with a disability, or caring for someone with dementia, Community Living Connections can help.

Community Living Connections provides anyone who calls a caring, highly-trained specialist who will give them easy access to information, individual consultation and service options. Often, we can connect people with services close to their home, reducing stress and travel time. Staff can also help you determine if you're eligible for programs, services and public benefits to help you or your loved one live with dignity and enjoy the best quality of life possible. All calls to the Community Living Connections line are free and confidential.

Information and Assistance/Referral

Staff will provide you information about programs and services close to your community, and assist you in accessing services such as:

  • Medicaid
  • State and federal benefits
  • Nutrition programs
  • Family caregiver programs
  • Kinship care
  • Care coordination
  • Medicaid case management
  • Minor home repair
  • Volunteer transportation
  • And many more

Individual consultation and support

Staff will conduct an interview and assist you in creating a plan based on your goals and values. They will also help you make an informed choice about the service and support options available to you. Staff will provide short term assistance in accessing services and planning for your long-term needs if you do not have someone to help you with these tasks.
