PRIVACY NOTICE: Seattle Fire Department will not use the information you provide below for any purpose other than requesting learning materials and/or registering for a Firefighter visit. You may be contacted by the Seattle Fire Department Public Education Division if you indicate that you want to schedule a Firefighter visit, if there is a question regarding your registration or a scheduling conflict for the dates you select for the Firefighter visit.

     and can make copies of the materials for the students. (You can also

YES! I want to borrow a Safety Suitcase. I understand that the Seattle Fire
     Department School Program Coordinator will contact me to arrange pickup of
     the Suitcase. I understand that I may borrow the Safety Suitcase for two
     (2) weeks at a time and that I must pickup, and return, the Safety Suitcase
     in person at the Seattle Fire Marshal's Office.

Firefighter Visit Request (Optional)

   If you DO NOT want to schedule a Firefighter visit then skip past this
   section and submit this registration form, otherwise, select a visit option
   below and complete the rest of this form before submitting your
   registration.

We would like to visit and tour a Seattle Fire Station
      Important: Enter the No. of the Fire Station you would like to visit
           Note: If you are unsure of the station number you wish to tour, please visit the
               Fire Stations page for list of all  Seattle Fire Stations and their street addresses.

We would like to have a firefighter crew visit our building at the address
above and agree to provide a parking area for the fire apparatus.

Please indicate three possible dates for a Station Tour. You will be contacted to confirm which of the dates listed will work best. Be sure each date is at least 4 weeks from the date you are submitting this form.

NOTE: The firefighter visit may be cancelled at the last minute with no notification due to emergency response or firefighter training requirements.

with the firefighters and my registration complies with the rules and
conditions.
