These optional forms may be used by certified technicians as a template to provide documentation of their system test results to building owners. System test results must be submitted electronically to http://www.thecomplianceengine.com/.  Seattle Fire must always be notified of impairments lasting more than eight hours.

The forms offer a general guideline for the type of testing and results that should be documented. However, certified technicians and the companies they work for should ensure testing procedures and documentation must meet requirements established in national standards including NFPA 25 and NFPA 72.
