What’s Changed with the Online Request Form?

What’s new?
As part of the City’s commitment to open communications with our constituents, we have implemented a new application for your service requests.

Why did we change to this form?
The new Customer Service Request (CSR) application provides a more efficient way for you to submit -- and for us to track – your requests for service, inquiries and/or complaints.

What are some of the new / improved features?

  • On-line access to the status of your inquiry, if you have provided your contact information.
  • Ability to become a registered user, allowing you to set up templates for regularly accessed services.
  • More online forms for specific City services, including SDOT, SPD, SPU and Law
  • Access to City FAQs and Knowledgebase features through the web portal, with more enhancements coming in 2012.
  • Streamlined intake process for customers and staff.
