For-Hire Vehicle Licensing Process

The Consumer Affairs Unit performs for-hire vehicle licensing for the City of Seattle. The primary licensing duty consists of the yearly renewal of every for-hire vehicles license. Renewing of the license occurs May 16th through June 30th. 

Licensing of a for hire vehicle requires many processes to be completed before the actual licensing of the for-hire vehicle is performed at the Consumer Affair Unit, Test Station.

The required original forms for for hire vehicle licensing documentation are:

1) Vehicle Registration. Obtained from the State of Washington, Department of Licensing. Must include legal name, DBA (doing business as), vehicle number.. (Example = Seattle For Hire, #1234, John Smith) Approved Vehicle Types.

2) Insurance Binder or Policy. May be emailed by insurance agent to vehicleinsurance@seattle.gov. Must include legal name, DBA (doing business as), vehicle number.. (Example = Seattle For Hire, #1234, John Smith)

3) Purchase State Master License. Must provide UBI number to Consumer Affairs Unit. May be emailed by State DOL to associationreports@seattle.gov. Must include legal name, DBA (doing business as), vehicle number.. (Example = Seattle For Hire, #1234, John Smith)

4) State For Hire License. Must provide UBI number to Consumer Affairs Unit. May be emailed by State DOL to associationreports@seattle.gov. Must include legal name, DBA (doing business as), vehicle number.. (Example = Seattle For Hire, #1234, John Smith)

5) Certificate of Mechanical Safety. Obtained from an city approved ASE certified mechanic.

6) For Hire Vehicle License Application. Obtained at Consumer Affairs Unit or Fill out here and print version.

7) Rate Filing Form. Obtained at Consumer Affairs Unit

8) Zone Fare Map. Must be provided to Consumer Affair Unit

9) Security Camera Installation Form. Obtained from an city approved camera installer. (only city or dual licensed vehicles).

10) Original LLC or INC Documentation. Obtained from State of Washington, Master License Department. (if applicable) NOTE: State LLC or INC paperwork not acceptable if application process is performed online. A manual state stamp is required on the state LLC oe INC documentation. A visit to Olympia Washington is required for State LLC or INC documentation being used for for hire vehicle licensing. The State of Washingon, Master License Department may email appropriately office stamped LLC Amended Report, Profit Amended Report or LLC License Renewal & Annual Report to the Consumer Affairs Unit to associationreports@seattle.gov.

11) Criminal Record Check/Fingerprinting. Fingerprint cards and instructions are obtained from the Consumer Affairs Unit. Criminal Record Check for License Application (only city & dual licensed vehicles)

*** The documents below are only for an entirely new for hire company. Not for a person/vehicle who is affiliating with an existing for hire company ***

A) Request for Acceptance of Color Scheme and Trade Name for For-Hire Vehicle Business. This form has to be approved by the Consumer Affairs Unit prior to licensing documentation above.

B) Initial For Hire Company Information Form. Obtained at Consumer Affairs Unit.
