Application for Emergency Communications Team Membership & State Emergency Worker Registration

Instructions:

  1. Please select the organization for which you wish to apply.
  2. Complete the form.
  3. Finally, click the "Send Application" button to e-mail it to the EOC.
  4. Following submission of your application, you need to visit the Seattle Police Department so they can take your finger prints and conduct a records check.
    • Please go to the Justice Center, Police Headquarters, located at 610 5th Ave. (at the corner of 5th Ave. and Cherry St.)
    • Hours are Monday through Thursday - 8:30 a.m. until 4:00 p.m.
    • Enter the most northern door of the building on 5th Ave. and walk up to the window. Let the staff know you are there for finger printing.
    • It is most important to tell them that you are applying to be a volunteer at the Seattle EOC. If they don't know what to do, ask them to write Maris Catalano's name on the finger print card and route it to her. She is an SPD ID Supervisor and is familiar with the ACS program.
    • E-mail Mark Sheppard at Mark Sheppard to confirm you have completed this step.
  5. Once the background check is complete, you will be contacted to have your picture taken for your ID badge.
  6. Upon completion of the process, your ID will be mailed to your residence.
